Start by enrolling in the cooperative:
Cost of membership in the Cooperatives: $15 Per Household Per Month and one-time registration fee of $30 Per Household.
then:
Choose a member Physician whom you can pay monthly for Primary Care Medical Services.

• $18 per adult 18-64
• $18 per child 00-17
• $35 per adult 65+

Choose Diagnostic Imaging Services (MRI, CT, Pet, Ultra Sound and X-ray) if desired

• $15 per adult 18+
• $10 per child 00-17

Know that all medical information between the doctor and the patient is strictly confidential.

Basic membership also includes discounted Dental and Vision and Pharmacy services

Membership in PPC includes a prescription drug card (through which brand name and generic drugs are priced at 15% to 50% below wholesale).

Members have access to over 300 specialist physicians in Linked IPAs at Reduced Rates (30% to 50% below usual medical fees).

Patient members also have access to true group health insurance (no pre-existing conditions exclusions underwritten by New Era Life).

The basic plan is a $25,000 annual limit indemnity policy with payments based of a fee schedule contracted by PPC with its member providers.

The monthly cost for non tobacco users is:
• $106 for adults ages 18-44 and
• $205 for adults ages 45-64 and
• $594 for ages 65-74.
  The monthly cost for tobacco users is:
• $118 for adults ages 18-44 and
• $228 for adults ages 45-64 and
• $660 for ages 65-74.

  The monthly cost for each child 0-17 is: $60.
  There is $20 one time policy fee.
  There is $25 office visit copayment.

  An employer sponsored $2,000,000 plan is also available through PPC if the employer pays at least 75% of the employee’s premium.

The monthly cost for non tobacco users is:

$181 for adults ages 18-44 and

$280 for adults ages 45-64 and

$669 for ages 65-74.

The monthly cost for tobacco users is:

$193 for adults ages 18-44 and

$303 for adults ages 45-64 and

$735 for ages 65-74.

The monthly cost for on or more children 0-17 is: $160.
There is $20 one time policy fee.
There is $25 copay for office visits.

Patient Advocacy is provided through our Sponsoring organization, TBT, a non-profit tax exempt charity which arranges for medical services for patients without resources to obtain necessary health care.

Members are assisted in getting the right kind of care at a reasonable price and in a timely manner.
